What to bring?

• Please bring identification, insurance information and co-payment. We accept cash and check for co-payment.

• First time patients will also need to fill out our new patient form. If you would like to save some time when you come, please click here to download the form in PDF format. You may print it out and complete it before you come in.

What to expect?

• Please arrive 15 minutes before your scheduled appointment.

• You will be greeted as you come into the office, asked for your insurance information and to fill out our new patient paperwork.

• Before being seen by the Doctor turn off all cellular devices.

• On average, your first visit will last ½ hour to 45 minutes.

• Will the treatment hurt?

  • It is very rare that the treatment you receive will cause any type of discomfort. There are occasions that you will experience minor soreness after the treatment but it usually goes away in a very short amount of time.

• Do you accept credit cards for co-payment?

  • We only accept cash and check for co-payment.

• Do you work with HSA accounts?

  • We accept cash or check for your co-payment. We will give you a receipt after your payment. With the receipt you can present it to your HSA company and receive a reimbursement from them.
